The Lost Tower Tattoo Consent Form

I hereby consent to receive a tattoo from the artist named on this form at The Lost Tower Tattoo Studio. I understand and agree to the following terms and conditions associated with the tattooing process:

1.     Client Information​

     I will provide the following details and confirm their truthfulness and accuracy:

·       Full Name

·       Date of Birth

·       Address

·       Email

·       Phone Number

2.     Artist Information​​

              I have been provided with the following:

·       Studio Address

·       Studio Phone Number

·       Studio Name

·       Full Name

3.     Legal Requirements:

·       I am of legal age to receive a tattoo according to UK law. I understand that declaring I am under 18 is a fraudulent act and The Lost Tower may take criminal proceedings against me if I fraudulently claim to be over the age of 18.

·       I confirm that The Lost Tower and/or my artist have confirmed I am aged 18 or over and I have provided relevant identification in photograph format prior to my appointment taking place. I also confirm all photo ID presented to The Lost Tower and/or any member of its staff is a viable, legal ID and not a replica.

4.     Risks and Side Effects:​

·       I understand that there are inherent risks associated with receiving a tattoo, including but not limited to infection, allergic reactions, scarring, and undesirable changes in the appearance of the tattoo over time. I understand tattooing is not an exact science and issues can arise that are completely out of the tattoo artists control, so cannot hold the artist responsible for any issues after the tattoo process is complete.

·       I acknowledge and accept full responsibility that the tattoo procedure may cause some pain, swelling, and discomfort, and there may be temporary or permanent side effects.

·       I am not under the influence of alcohol or drugs at the time of receiving the tattoo, as this can increase risks and impair healing.

5.     Use of Numbing Creams and Sprays:

·       I understand that topical numbing creams and sprays may be used to minimize pain during the tattooing process and are done so fully at my own discretion. The Lost Tower are not responsible for any creams, sprays or other topical agents applied to my tattoo before or during the process.

·       I am aware that the use of numbing agents may not completely eliminate discomfort, and I may still experience some level of pain.

·       I acknowledge that numbing creams and sprays may have their own associated risks and side effects, and I accept these risks and take full responsibility for any and all issues that arise from their use.

6.     Personal Health and Medical Conditions:

·       I have disclosed to the tattoo artist any pre-existing medical conditions, allergies, or medications that may affect the tattooing process or my ability to heal properly.

·       I understand that some medical conditions I have that I am unaware of can also have undesirable effects and accept full responsibility and liability for these.

·       I understand that certain medical conditions may make tattooing more risky and should consult with a healthcare provider before getting a tattoo.

7.     Aftercare:

·       I understand that proper aftercare is essential for the healing process. The Lost Tower has provided me with aftercare instructions, and I am responsible for following them diligently. Any issues that arise due to poor aftercare once I have left the studio are my responsibility and The Lost Tower bears no responsibility for the healing of my tattoo outside of providing advice.

8.     Tattoo Design:

·       I confirm that I have approved the design and placement of the tattoo and that any changes during the process will be discussed and agreed upon between me and the tattoo artist.

9.     Consent:

·       I consent to the tattoo artist tattooing the design agreed upon, as well as any adjustments deemed necessary during the process.

·       I acknowledge that I am receiving the tattoo voluntarily, without any coercion or pressure.

10.  Payment:

·       I understand and agree to the payment terms and pricing for the tattoo, as discussed with the tattoo artist.

·       If payment is not made as agreed, I understand that the Lost Tower may involve a debt collection agency to act on their behalf.

 

11.  Release and Waiver:

·       I release my tattoo artist named on this form, The Lost Tower Studio  and any associates from any and all liability for any complications or injuries resulting from the tattooing process and any of the above procedures mentioned on this form.

·       I accept full responsibility for the tattoo and its aftercare.

12.  Photography:

·       I grant permission for the tattoo artist to photograph the completed tattoo for their portfolio and social media, unless I specify otherwise in writing.

I have read and fully understand the contents of this consent form. I have had the opportunity to ask any questions and have received satisfactory answers. By signing below, I acknowledge and accept all terms and conditions.
